• VA Form 22-1990 (Application for Veterans Educatonal Benefits) **This application is for first-time applicants only. If you have previously filed this form, DO NOT complete it a second time.**
  
• Form 22-1995 Request for Change of VA Education Program or Place of Training **This form is to be used if you are changing majors or transferring from another institution or transferring from WVU to another institution**
  
• WVU Coordinator Card
  
• Photocopy or Certified Copy of DD-214 member copy 4 (discharge papers)
  
• Kicker Contract (Issued by unit)
